From 5th to 9th April 2011
  • Cook cum Helper not Engaged as per GOI norm.
  • Cook cum Helper not paid as per GOI norm
  • Inordinate delay in payment of Cook cum Helpers’ s honorarium
  • Kitchen Devices fund released by GOI in 2007 not disbursed yet
  • Supervision charges claimed from Cooking Cost
  • Meal not supplied since 13th March-3rd April in Kalama.
  • Delay in Release of funds by District to Schools(3 months)
  • Quality of Foodgrains not of FAQ in rural schools
  • Less Quantity of foodgrains delivered at school
  • Menu decided by State and not displayed in schools.
  • Storage of foodgrains in HM’s room/Class room in most of the schools.
  • Banned insecticides used in stored foodgrains
  • Health check up very irregular and health cards of children not maintained.
  • School Health Programme not reached to schools in Cuttack
  • Convergence with Health Department needs to be strengthened.
  • Date Expired IFA and De-worming tablets supplied in Mayurbhanj district.
  • Functionaries at district and sub district level unaware of Revised or old norms
  • Weak monitoring at district level and below
  • Children are supposed to bring plates from their home
  • Standard MDM register is not available
  • MME fund not utilized
  • SHG not maintaining their books and accounts properly
  • District Welfare Officer, Sambalpur has very limited knowledge of MDMS
  • No Dedicated staff for MDM at State/District and sub district level
  • Community awareness and Involvement is limited
  • Eligibility, Entitlement and Logo of MDM not found either in schools or at prominent places.
  • Toilets inadequate
  • Kitchen Sheds not available in 7 schools in Jharsuguda, Balasore and Bhapur block of Nayagarh
  • Grievance Redressal mechanism for MDM is missing at district level.
  • No External and concurrent evaluation of the programme
  • No fire extinguisher available in schools
  • Unhygenic cooking and serving
  • Green vegetables missing and Pulses not supplied in some schools in Mayurbhanj.
  • Tribal Welfare Schools boarding lodging condition not satisfactory.
